Catholics, when do members receive their Patron Saint names?

I am not Catholic. I have decided to give my two grandsons a gift certificate to their favorite store and a Patron Saint Medal for their Holy Communion gift.
My question is, when do they receive their Patron Saint name? Before Holy Communion or at Confirmation? If it is at Confirmation I'll have to think of something else to give them. Boys are so hard to buy for!

You take the patron saint name at Confirmation,when your about twelve. I remember I took Catherine.
